JOB DUTIES
Your responsibilities will include 50% case management and 50% care management. 
Responsibilities:
  • Perform comprehensive in-home assessments. This will include a detailed physical and psychosocial assessment, the compilation of a medication profile and client resources, and evaluation of the living environment. You will determine the status of the patient as well as the safety of the home and be instrumental in implementing fall precautions, proper use of necessary supplies/equipment, and any other relevant safety measures.
  • Develop a fully individualized plan of care based on your assessment.
  • Communicate with caregivers and the clients/families of the clients to stay up to date on any changes in their condition.
  • Update the Plan of Care accordingly to reflect/ensure progress towards goals and outcomes. 
  • Provide ongoing education and in-field support to HHAs and PCAs.
REQUIREMENTS:
  • Current License as a Registered Nurse (RN) in New York State
  • Knowledge of safety and infection control practices
  • Maintain confidentiality 
  • Ready to join a healthy, hardworking team environment!
WORK LOCATION:
  • Primarily in office (Greenpoint Brooklyn, NY)
SCHEDULE:
  • Monday-Friday
  • Full-Time
