We are looking for dependable, skilled, and patient-focused Licensed Vocational Nurses to join our growing home health team. This role is a great fit for LVNs who enjoy working independently while still being supported by an experienced care team.
What You’ll Do
- Provide direct nursing care in patients’ homes according to the plan of care
- Administer medications, injections, treatments, and wound care as ordered
- Monitor vital signs, symptoms, and patient progress
- Communicate changes in condition to the RN, physician, and care team as appropriate
- Coordinate care with the interdisciplinary team
- Complete accurate and timely documentation, including EMR documentation
- Follow infection control, safety, HIPAA, and agency compliance standards
- Support personal care needs within LVN scope of practice when appropriate
What We’re Looking For
- Current California LVN license
- Graduate of an accredited nursing program
- Current CPR certification
- Home health experience preferred
- Wound care and venipuncture experience preferred
- Comfortable with EMR documentation
- Strong communication, observation, and time-management skills
- Ability to work independently and coordinate with a care team
- Valid California driver’s license, auto insurance, and reliable transportation
