ComForCare in Maryville, TN is seeking a dedicated and experienced Registered Nurse (RN) to specialize in performing comprehensive initial assessments and developing patient care plans in the home setting.

This is an autonomous and vital role requiring strong clinical judgment and excellent communication skills. You will be the crucial link between the patient, their physician, and our care team, ensuring every client receives personalized, high-quality care from day one. Key Responsibilities
- Conduct thorough initial and ongoing patient assessments (physical, psychosocial, environmental) in the client's home.
- Develop, implement, and continually revise individualized Plans of Care in collaboration with the physician and interdisciplinary team.
- Provide essential patient and caregiver education on disease management, medications, and safety precautions.

Coordinate services to ensure continuity of care, including therapy, social work, and aide services.
- Maintain accurate, timely, and compliant electronic medical record (EMR) documentation.
- Monitor patient status and promptly report significant changes to the physician and clinical management.
Qualifications

License: Current, unencumbered Registered Nurse (RN) license in the state of Tennessee.

Experience: Minimum of one (1) year of recent clinical experience, preferably in Home Health or an autonomous setting.

Skills: Strong critical thinking, excellent documentation skills, and proficiency with EMR systems.

Requirements: Valid Driver’s License and reliable transportation.

Compliance: Ability to meet company background check standards and drug testing.
