ComForCare Home Care is excited to expand our mission by hiring a PRN RN to join our Team. Not every RN role has to mean long shifts and burnout! The nature of home care allows for one-to-one attentiveness. Rather than rushing from one case to the next, you will be able to take your time with each patient and provide exceptional care you can be proud of. The hours may range from 4-16+ hours per month, with many opportunities for meaningful involvement!
This role is ideal for someone who is team-oriented and open to learning as we build something different.

Requirements:
- Unencumbered New York State RN License
- CPR, BLS Certification
- 2 or more years of hospital experience (Acute Care) and/or Home Care
- Ability to perform assessments in Brooklyn and Manhattan, occasionally Queens and the Bronx
- Willingness to be part of a growing Team

Duties:
- Perform comprehensive in-home assessments
- Develop or update a fully individualized plan of care based on your assessment
- Perform Supervisory visits with established HHA/PCA and Client matches
- Communicate with the office, caregivers, and clients/families, to ensure everyone is updated on any developments or needs
- Provide ongoing education and in-field support to HHAs and PCAs
