With ComForCare, clients can live independently and continue to do the things they love.  As a Registered Nurse at ComForCare, you will conduct thorough client assessments, create personalized care plans, implement interventions to achieve client goals, update care plans to track progress, administer medications when necessary, maintain detailed client records, and coordinate referrals to other healthcare professionals as required.

We are seeking a compassionate and skilled Registered Nurse (RN) to provide one-on-one private duty nursing (PDN) services in the home setting. The RN will deliver continuous, skilled nursing care to medically complex patients in accordance with New Jersey Medicaid/MLTSS requirements, physician orders, and ACHC accreditation standards

What we are looking for: 
  • Graduate of an accredited school of professional nursing; BSN required.
  • BSN in nursing and two years combined public health nursing and progressive professional responsibilities in public health nursing. 
  • Current license in good standing as a Registered Nurse in the state of New Jersey. 
  • Minimum of two (2) years nursing experience in an acute hospital setting or equivalent experience with one year of supervisory experience.
  • Demonstrated ability to supervise and direct clinical and administrative personnel.
  • Complies with accepted professional standards and practice.
  • Excellent verbal and written communication skills and strong interpersonal skills.
  • Knowledge of home care federal and state regulations.
  • Knowledge of reimbursement sources and documentation requirements within home care.
  • Must be a licensed driver with automobile insured in accordance with Agency and state requirements.
  • Has current CPR certification.

What you will be doing:
  • Provide direct, continuous skilled nursing care to adult patients in the home. 
  • Perform and document comprehensive nursing assessments and ongoing reassessments. 
  • Implement and follow an individualized Plan of Care (POC) approved by the physician. 
  • Administer medications, treatments, and therapies per physician orders. 
  • Monitor and manage complex medical conditions (e.g., tracheostomy, ventilator, G-tube, seizure management).
  • Maintain accurate, timely, and compliant clinical documentation in accordance with ACHC and NJ regulations. 
  • Educate and support patients and certified home health aides (CHHA) regarding care plans and treatments. 
  • Communicate effectively with physicians, care managers, and interdisciplinary team members.
  • Participate in case conferences, supervisory visits, and quality improvement activities.
  • Ensure care is delivered in accordance with infection control and patient safety standards.

Hourly Range:
  • 32.00-40.00 per hour based on years of experience.
 
Compensation: $32.00 - $40.00 per hour
