Clinical Director (Registered Nurse) – Home Care

Position Summary
The Clinical Director (RN) is responsible for overseeing all clinical operations and ensuring the delivery of high-quality, compliant skilled nursing services in the home setting. This role provides leadership, supervision, and clinical guidance to Registered Nurses and caregivers, while ensuring adherence to Georgia state regulations and best practices in home care.
The Clinical Director serves as the clinical leader of the organization—responsible for care standards, staff competency, client outcomes, and the development of the skilled nursing service line.
Key Responsibilities
Clinical Leadership & Oversight
- Provide clinical supervision and direction to all nursing staff (RNs, LPNs, and other clinical personnel as applicable)
- Establish and enforce clinical policies, procedures, and standards of care
- Serve as the primary clinical resource for complex cases and escalations
- Ensure care delivery aligns with physician orders, scope of practice, and regulatory requirements
Staff Management & Development
- Recruit, hire, onboard, and train nursing staff
- Conduct competency evaluations, skills validations, and performance reviews
- Provide ongoing coaching, mentoring, and corrective action when necessary
- Ensure all clinical staff maintain appropriate licensure and certifications
- Develop staffing models and ensure adequate clinical coverage
Clinical Operations
- Oversee client assessments, reassessments, and care plan development
- Review and approve care plans for accuracy, appropriateness, and compliance
- Ensure timely and accurate clinical documentation
- Monitor clinical outcomes and implement quality improvement initiatives
- Participate in client case conferences and interdisciplinary coordination
Quality Assurance & Compliance
- Ensure compliance with Georgia Department of Community Health (DCH) regulations and any applicable licensing requirements
- Maintain readiness for audits, surveys, and inspections
- Develop and manage Quality Assurance and Performance Improvement (QAPI) activities
- Investigate incidents, complaints, and adverse events; implement corrective actions
- Ensure adherence to infection control, safety protocols, and HIPAA standards
Client & Family Engagement
- Participate in client intake and high-acuity case evaluations
- Address client and family concerns related to clinical care
- Support care transitions, including hospital discharges and hospice coordination
Business & Program Development
- Lead the development and expansion of skilled nursing services (e.g., IV therapy, chronic disease management programs)
- Collaborate with leadership on strategic growth initiatives
- Build relationships with referral sources (physicians, hospitals, case managers)
- Support marketing efforts with clinical expertise when needed
Qualifications
- Active, unrestricted Registered Nurse (RN) license in Georgia
- Bachelor of Science in Nursing (BSN) preferred
- Minimum 3–5 years of clinical nursing experience, with at least 1–2 years in home health or home care
- Prior leadership or supervisory experience strongly preferred
- Demonstrated expertise in:
- IV infusions
- Wound care
- Chronic disease management
- Clinical assessments and care planning
- Strong knowledge of Georgia home care regulations and compliance requirements
- Excellent leadership, communication, and organizational skills
- Ability to balance administrative duties with field-based clinical oversight
- Valid driver’s license and reliable transportation
